Dorzolamide, a carbonic anhydrase inhibitor, primarily acts by reducing the production of aqueous humor in the eye. It inhibits carbonic anhydrase, an enzyme responsible for the formation of bicarbonate ions, which are essential for aqueous humor secretion. By decreasing aqueous humor production, Dorzolamide lowers intraocular pressure. Timolol, on the other hand, is a beta-adrenergic antagonist that works by reducing the production of aqueous humor and enhancing its outflow. It blocks beta-adrenergic receptors in the ciliary body, inhibiting the action of beta-adrenergic agonists, which are involved in aqueous humor secretion. This dual mechanism of action, combined with the beta-blocking effect of Timolol, results in a more effective reduction of intraocular pressure in conditions like ocular hypertension and open-angle glaucoma.
